Delayed re-formation of anterior chamber after trephine operations.

چکیده
DELAYED re-formation of the anterior chamber is a frequent complication of trephine operations for the relief of glaucoma, especially Elliott's operation. This complication is a very serious one which, if not relieved, leads to loss of the eye. Spaeth (1944) discusses this complication but does not describe a treatment for it. Stallard (1946) recommends injection of normal saline into the anterior chamber through the trephine hole under a conjunctival tunnel. Duverger and others (1950) advise cauterization of the conjunctiva with actual cautery. Neither method has been successful in our hands. Cauterization with actual cautery leads to perforation of the conjunctiva and fistulization. Stallard's procedure fails because the normal saline regurgitates under the flap and is resorbed. We have tried a simple technique which may be carried out under local anaesthesia. either alone or in addition to Stallard's.

Penetrating corneal graft; orientation of the initial penetration of the trephine into the host cornea.
DURING the operation of perforating graft of the cornea, the trephine in the great majority of cases enters the anterior chamber in only a portion of its circumference, the remaining circumference of the graft being completed with scissors. The author has noticed, both in operations performed personally and in those performed by others, that the portion of the circumference of the trephine whic...

Failure of filtration operations in the African.
Filtration operations in Negroes and in other heavily-pigmented races have earned a bad reputation for a high rate of failure, but it is difficult to estimate the extent of this failure because it is difficult if not impossible to achieve an adequate follow-up in these cases (McNair, I95I; Welsh, I969). Iliff (I944) reported a success rate of 23 per cent. with trephine operations and of 50 per ...

THE occurrence of cataract in association with glaucoma presents a therapeutic problem of some magnitude which has attracted a good deal of attention in recent years (Samuels, 1947; Sourdille, 1950; Chandler, 1949;, Fran9ois, 1947; Williamson-Noble, 1953). The difficulties of treatment are not made less by the uncertainty which still exists as to the role played by the glaucomatous process and ...

Trantas (I 907) made the first recorded observations on the angle of the anterior chamber, which he viewed through an ophthalmoscope while indenting the sclera at the limbus on the opposite side. He introduced the term gonioscopy, and found that a view could be obtained using either direct or indirect ophthalmoscopy. A few years later the first attempt to view the angle through a contact lens w...
